The May 16th deadline for Nominations of the 2011 Joseph L. Galletta Spirit of Recovery Award is fast approaching!

To nominate someone simply follow these steps:
1.  You'll need Adobe Acrobat Reader.

3.  Download the form.
4.  Fill out the form and email or fax completed form to HVRC
